Birth Certificate- must be 5 years by September 1, 2011

Record of Immunizations to date- physical and dental exams are required by the State for entry into Kindergarten.

Parent's proof of residence and photo ID- ex. school tax, county tax, current drivers license, PA photo ID, Military ID
