COVID-19 Self-Reporting for Employees

CASD Health & Safety Plan

As outlined in our board-approved Health and Safety Plan, any employee who is unable to report to work due to COVID-19 must complete the CASD COVID-19 SELF-REPORTING FORM. This is a critical step in maintaining a safe and healthy environment for all staff and students.

Reminder for Supervisors: If an employee informs you that they have tested positive for COVID-19, please remind them to complete the CASD COVID-19 SELF-REPORTING FORM immediately.

Important: When a CASD school nurse advises an employee to isolate for a specific number of days due to a positive home COVID test or symptoms, the employee must obtain a medical excuse if they are absent for 3 or more consecutive days, in accordance with employee contract guidelines.

Procedure:  
1. If an employee tests positive for COVID-19, they must complete the CASD COVID-19 SELF-REPORTING FORM.
2. The employee should use their available sick days for their absence.
3. If the absence extends beyond 3 days, a medical excuse must be provided.
