Dental Exams are required for students in grades K/1, 3 & 7.  These exams may be completed by your family dentist, or you may request the use of the Mobile Dentists.  Be sure to select the correct school.

Physical Forms

Physical Exams are required for all students in grades K/1, 3, 6 & 11.  These exams may be completed by the school doctor who visits annually, or by your primary care provider.
