School Health Services are available to all students in the Catasauqua Area School District.  Nurses are on duty in all buildings during the school day and accessible to all students.  

Growth and vision screening are completed annually for all students.  Additionally, hearing screening is completed in grades K, 1, 2, 3, 7 & 11 and for other grades as needed.  Scoliosis screening is completed in grade 6 & 7.  Parents/Guardians will be informed for all students with results outside the normal limits. 

Physical Exams are required for all students in grades K/1, 3, 6 & 11.  These exams may be completed by the school doctor who visits annually, or by your primary care provider.  For students in 11th grade, a sports physical or copy of the learner’s permit for driving is acceptable.

Dental Exams are required for students in grades K/1, 3 & 7.  These exams may be completed by your family dentist, or you may request the use of the Mobile Dentists.  Mobile Dentists forms can be submitted on paper or through their website at www.MySchoolDentist.com.  Be sure to select the correct school.

All students are required to be compliant with immunization requirements on the first day of school.  There is only a very brief grace period so please be sure to submit all immunization information to your school nurses as soon as possible.  Details can be found in your child’s Parent/Student Handbook or on the PA DIvision of Immunization website. 

Medications: Students are not permitted to carry medications with them with the exception of inhalers, epinephrine autoinjectors and insulin pumps.  The Medication Authorization form - signed by the primary care provider and the parent/guardian - is required for the distribution of all medications during the school day.  All medication paperwork must be kept on file in the school nurse’s office and renewed annually.  No medication will be distributed without the appropriate paperwork on file.  Please see the Medication Authorization form, your school’s Student/Parent Handbook and Medication Policy for more details.  

Please review and update your child’s Health Information and Emergency Contacts on PowerSchool at least annually.  These forms are important to maintain updates to your child’s contact information in case of emergencies.  If you need to add more contacts than the space allows, please contact your school nurse directly.
