Sports physicals are required annually if your student wishes to participate in school sponsored activities.  These physicals can be done by your private health care provider or you may request that your student see the school doctor. 

Additionally, it is required that your student has proof of having had a tetanus shot within the ten year period prior to beginning to participate in the activity. 

A code of conduct form must be signed by the student and the parent/guardian and on file in the office of the Activities Director.
